Cornerstone Incident/Accident Report Form

Medical Form

Children are a blessing from God, endowed with dignity by virtue of being created in the image of God.  Furthermore, these children are particularly vulnerable, needing protection, care, and godly instruction from parents, guardians, family, and the local church.  In this way, God has entrusted the children of our congregation to our care.
The following information must be completed each time an individual is injured or is involved in an incident of concern.  The senior leader of the age group (CMD for children birth-6th Grade, Associate Pastor for children 7th-12th Grade, for example) must be informed of all of the details of the incident.
In an effort to consolidate paperwork and to minimize confusion on events, we are asking you to complete the form below. Should pertinent information change (like insurance companies or allergies) please inform our office. It will be important for you to fill out a permission slip for major outings; we will provide these for you.

Event & Facilities Request Form

Reminder: If you or any ministry leaders you work with are planning an event that requires the use of church facilities, please complete the Event & Facilities Request Form using the button below. 

Reimbursement Form

If you have purchased items for a church ministry or event and need to be reimbursed, please complete the Reimbursement Form and submit it along with your receipts. This helps us keep clear records and process your reimbursement quickly.

In this way, God has entrusted the children of our congregation to our care. This is a task that carries tremendous reward, but one that also carries tremendous weight and responsibility.
Parents give written permission if their student rides with another student driving.
